To enroll for classes, please print and complete the following forms:
- Liability Waiver
- Medical Emergency Consent Form
- Student Enrollment Form
- Payment Contract (Printable)
- Payment Contract (Fillable, Excel Format)
Complete all forms (a copy of your medical insurance card may be made at MBA on your first visit). The tuition rates can be found on the Class Offerings and Tuition Rates Page. Choose your payment option – year, semester or month. The monthly payment option requires a contract for five payments (1- semester) which must be made with a recurring charge to your credit/debit card. The first month’s tuition will be charged at the time of enrollment, with the subsequent monthly payments beginning in September.
Pre-paid Tuition will be refunded or a monthly payment contract rescinded only for a long-term medical condition that forces the student to withdraw from the program. This medical condition must be documented by a physician.
Scholarships are applicable only for student contracts for the complete year or for 10 monthly payments. If a contract is broken for a documented medical condition, the scholarship will not be reinstated if the student re-enrolls later in that same school year.
Discounts are applicable only for student contracts for the complete year or for ten monthly payments. If a contract is broken for a documented medical condition, any discounts applied to the preceding payments must be refunded to the Michigan Ballet Academy.
